Job Summary
 

The Transition Counselor ensures successful transition to community integrated employment opportunities for program participants with disabilities. Assesses referred participants for job readiness and facilitates transition into appropriate employment. Works closely with employer partners to assess career opportunities and integrate participants into the workplace.



Responsibilities

1. Works with Work Activity Program and Supported Employment staff to identify participants that are/may be ready for integrated employment.
2. Assesses and prepares participants through job readiness activities including interviewing strategies, resume writing, cover letter development, teamwork, and networking through one-on-one or group coaching.
3. Co-case manages participants that have been referred for integration, ensuring timely and accurate case file documentation. Ensures completion of all necessary documentation as required by funders, CARF, and organizational standards.
4. Engage employers to develop employment opportunities and retention for people with developmental disabilities.
5. Plans marketing and outreach services to promote employer awareness of potential qualified participants.
6. Researches and maintains current job listings and develops resources for people with developmental disabilities to secure employment related to their identified vocational goal.
7. Serves as advocate and liaison for people with disabilities utilizing employer incentives designed to help applicants with disabilities.
8. Collaborates with DOR counselors to assist participants with achieving goals established by DOR. 9. This position may require some travel up to 75% of the time; a valid CA driver's license and state- required auto insurance are necessary. Driving records must be acceptable by the company’s insurance carrier



Qualifications

• Minimum 4 years of case management experience required.
• Bachelor's degree from a four‐year college or university in Vocational Rehabilitation Counseling, Psychology, Social Work, or a related field required.
• Master’s degree preferred.
• Ability to work with minimal supervision.
• Certified Rehabilitation Counselor (CRC) highly desirable.
 


Experience

• Non‐profit experience preferred.
• Experience serving persons with disabilities; ADA knowledge required.
• Ability to speak another language and understand another culture as required by the program or local community.
